4th CEREBROVASCULAR GRAND ROUND Webinar, 07 April 2021
The 4th chapter of the great Cerebrovascular Grand Round Webinar, held under the auspices of the MENA STROKE Organization, American Stroke Association, and World Stroke Organization, and supported by Boehringer Ingelheim and Medtronic, was an informative session on Carotid Artery Stenting, Carotid Endarterectomy, and Traumatic Carotid Dissection. It revealed the updates in diagnosis and treatments of the condition. The event was noteworthy because it attracted 804 delegates from 34 countries around the world.
Dr. Suhail Abdulla Alrukn, President of MENASO and Consultant Neurology at Rashid Hospital, UAE, has appointed Dr. Adel Alhazzani, FRCPC, FAAN, Vice President, MENASO and Associate Professor Of Neurology at College of Medicine, King Saud University, KSA, Dr. Abdulrahman Aldakkan, Assistant Professor and Consultant Neurosurgeon Cerebrovascular and Endovascular Neurosurgery at Department of Surgery, King Saud University, KSA, and Dr. Fahad Essbaiheen, Assistant Professor and Interventional Neuroradiologist at Department of Medical Imaging, King Saud University, KSA as the Webinar Chairman.
Dr. Adel Alhazzani has delivered the opening remarks and welcomed the panel of chairman and speakers. Dr. Abdulrahman Aldakkan, who has also presented a lecture, introduced the other speakers, Dr. Julian Spears, Head of Division of Neurosurgery at St’ Michael’s Hospital, University of Toronto, Canada, and Dr. Musaad Alhamzah, Assistant Professor and Consultant Vascular Surgeon at Department of Surgery, King Saud University, KSA to the delegates.
Dr. Julian Spears’ lecture was focused on Carotid Artery Stenting and evidence based review of its current role. During the presentation, he reviewed the scientific basis for Carotid Artery Stenting in symptomatic and asymptomatic extra-cranial carotid disease, the influence of operator volume and outcome in carotid revascularization, and current system for rapid triage and Rx of symptomatic carotid disease. He detailed the studies with the help of 4 case presentations.
Dr. Musaad Alhamzah, has emphasized on Carotid Endarterectomy. He examined each and every aspect of the condition including epidemiology, indications, pre-operative evaluation, perioperative stroke management, complications and results and trends. He stated that Carotid Endarterectomy is durable procedure with an established benefit in treating symptomatic carotid lesions. According to him, careful selection of asymptomatic patients is very important. The registries data shows that Transcervical Carotid Artery Revascularization (TCAR) is showing superior results when compared to transfemoral approach.
Dr. Abdulrahman Aldakkan, has sought delegates’ attention to the lecture on Traumatic Carotid Dissection and discussed primarily on imaging and the controversies in management of the condition. Carotid dissection is a rare entity with an estimated incidence of 2-3 per 100,000 populations. According to etiology it can be classified as spontaneous and traumatic and as extracranial and intracranial depending on the location. He revealed that around 4% of carotid dissections are related to severe trauma and it can be occurred secondary to blunt trauma to the head and neck or rapid movement of the head in relation to the neck in any axis. According to him, high index of suspicion and early detection is the key.
